How Do I Order?

To download an order form (PDF format), just click here.

Fill in the information requested and then print the Order Form.  You then have 4 options:

1. Mail me the completed form,
2. Fax me the form,
3. Scan the form and then email it to me; alternatively,
4. You could provide all the information asked for in the Order Form in an email to me at Contact Mike!

Payment Options

1. Send a cheque, money order or credit card information to me by mail. (Cheques and Money Orders are to be made payable to Jostens).
2. Provide credit card information to me over the phone.
